Dear Parents of Lubbock-Cooper ISD Athletes:

In order to prepare our students for the upcoming school year, there are several required forms and procedures that need your attention. The UIL Physical and Medical History form must be completed by a physician to clear your child for participation:

**Before you begin please have your athlete’s school ID number available as it is required on each form**

Submit the following two items together to the designated athletic trainer or coach.

1.UIL Physical & Medical History Form
You may print the UIL Physical/Medical History Form under the “DOWNLOAD AND PRINT” tab at the top of this page, or request one from an Athletic Trainer. All LCISD student athletes/band members must have an updated physical to be able to participate. The physical must be dated AFTER April 1, 2025 to be accepted for the 2025-2026 school year. Please do not send through email! Physical forms should be turned into an Athletic Trainer, if not uploaded to RankOne.

2. Electronically submit the LCISD Online Physical Athletic Paperwork
LCISD requests that students and parents complete all Athletic Paperwork electronically. The UIL athletic forms are located under the “ELECTRONIC PARTICIPATION FORMS” tab at the top of the page, and will need to be dated after April 1, 2025. To sign the document, click inside the signature box and hold your mouse down. This will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click the refresh icon next to the signature box. If you are using a smart phone or device, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus. The forms may be submitted at any time, regardless of when the physical is scheduled. The dates of the UIL physical/medical history form and electronic form do not have to match. Once this section has been submitted and accepted, you will receive an email and may choose to print for your records.

A STUDENT WILL NOT BE CONSIDERED CLEARED FOR ANY ACTIVITIES UNTIL ALL FORMS LISTED ABOVE ARE COMPLETE AND SUBMITTED.
